Iodine: Properties, Uses and Dangers

Iodine: Properties, Uses and Dangers Introduction: Iodine is an indispensable micronutrient required in small amounts for the normal physiological function of the human body [1]. Iodine is a critical component of the thyroid hormones, which are necessary for various metabolic and enzymatic processes. These processes include control of the body’s metabolic rate, growth and development, neuron function and development. The recommended dietary intake for adult men and non-pregnant women is 150  µg/d, 220-250  µg/d for pregnant women and 250-290  µg/d for breastfeeding women (American thyroid Association) [2]. Seafood, dairy products, and plants grown in iodine-rich soils are decent sources of iodine as food [3]. Most other foods contain low amounts of iodine so individuals require additional sources to meet the recommended amounts. Insufficient intake of iodine results in a variety of disorders referred to as iodine deficiency disorders (IDD). They include mental impairment, goiter (enlargement of the thyroid gland), hypoth yroidism, and dwarfism [1-3]. IDD is especially destructive during the early stages of pregnancy and in early childhood. In their most severe form, IDD include cretinism (extreme case of neurological damage from fetal hypothyroidism), stillbirth and miscarriage, and increased infant mortality. IDD is a noteworthy public health problem in more than 50 countries. According, to the World Health Organization (2004) [4] an estimated 2 billion people worldwide (35.2% of the world population) suffer from inadequate iodine intake. Iodide is crucial to thyroid function in vertebrates, with vital implications for human health. It is important to recall that more than 95% of total iodine necessary for humans is accumulated in the thyroid gland. The history of therapeutic use of iodine dates back centuries. It has been described as the most potent antiseptic available. However, its therapeutic uses require careful evaluation because of its narrow range of intake to support optimal thyroid function [5]. One of the most notable features of iodine as an antiseptic is the lack of selection of resistant strains. Only one report of iodine resistance has been published [6]. The behaviour of iodine-based antiseptics on skin was investigated by Gottardi in 1995. Iodine antiseptics have wide scope of antimicrobial activity, killing all principal pathogens and given enough time even spores. Iodine based preparations and iodine salts are widely used as antimicrobial remedies, especially in the form of alcoholic solutions. This is due to the broad antimicrobial activity of iodine and the low cost of iodine components [7]. However, there is considerable controversy about the maximum safe iodine and duration of use. Povidone-iodine prepara tions are widely used as an antiseptic to prepare patient’s skin before surgery and are also used by surgeons and theatre staff as a skin cleaner and antiseptic in preoperative hand scrubs [8]. Many other wound dressing material containing iodine are Inadine, Iodosorb, Iodoflex, Iodozyme, Repithel etc. [9]. The antimicrobial property of iodine has been using for water disinfection since First world war to disinfect drinking water for troops in France and subsequently, US army during second world war used Globalin (tetraglycine hydroperiodide) tablets [10]. Iodine based disinfectant has been in use by NASA in space flights [11]. Today iodine based disinfection materials are mainly available in two forms, as iodine solution and iodine resin [12]. Iodine resins are solid-phase iodine disinfectants. Iodine resins are generally regarded as demand release disinfectants [12]. Starting in the early 1990’s increasing interest in the chemistry of polyvalent organic reagents is also notable and since then an innumerable of reports on the use of such compounds in organic synthesis have been published [13]. Wijs reagent, a solution of iodine monochloride in acetic acid and can be prepared from a mixture of iodine and iodine trichloride, is used for the estimation of the iodine value of fats and oils [14]. However there are various problems associated with the use of free iodine, like excess iodine ingestion cause thyroid disorders, irritation of tissues and short duration of action when used as antimicrobial remedies and high degree of instability [15]. These problems were overcome by the development of iodophores. Iodophores are complexes between iodine and a solubilising agent or carrier to increase the solubility and endure the release of iodine. In an aqueous iodophore solution, iodine is present in the form of different thermodynamically stable anionic iodine species and diatomic iodine [16]. Anionic species of iodine could interact with cationic groups of various polymers and form polymeric iodophores as ionic complexes. Four types of carriers have been generally used:- i) Polyoxymer iodophores. ii) Cationic surfactant iodophores. iii) Non-ionic surfactant iodophores. iv) Polyvinyl-pyrrolidine iodophores (also known as povidone iodine or PVP-I). In most of these carriers iodine is present in aggregates (or micelles) of surfactants, which act as reservoir of iodine. Polyoxymer iodophore are complexes of non-ionic copolymers with iodine. In case of non-ionic surfactant iodophore the complex formation take place through donor-acceptor mechanism with the surfactant ethereal oxygen (as donor) and iodine (as acceptor) [17]. PVP with iodine forms a stable charge-transfer complex. In PVP-I solutions, free species of iodine are formally controlled by the mass action law including a coupled reversible interaction between iodine–iodide, triiodide–polymer and iodine–triiodide–polymer complexes [18]. In 1981, Cadexomer iodine was developed as another means of delivering benign iodine. Cadexomer is a derivative of dextrines (containing some number of carboxyl groups) cross-linked with epichlorohydrin and exits in the form of water-insoluble microbeads; 0.9% of molecular iodine is physically (not chemically) tra pped in the core of these microbeads [19]. Solubility of elemental iodine increases in the presence of iodide ions, such as potassium iodide, where iodine reacts to form tri-iodide ions. Aqueous solutions of iodine are not stable and, depending on conditions, many different species may be present. Of these, it is believed that molecular iodine (I2) has the highest antimicrobial potential. Stability is influenced by pH and activity diminishes with increased alkalinity and storage time [20]. The seven principal iodine species found in aqueous solution are I2, HOI, OIà ¢Ã¢â‚¬ Ã¢â€š ¬, H2OI+, I3à ¢Ã¢â‚¬ Ã¢â€š ¬, Ià ¢Ã¢â‚¬ Ã¢â€š ¬, of which only hydrated iodine (I2), hypoiodous acid (HOI) and iodine cation (H2OI+) possess bactericidal activity. At physiologically compatible pH and low concentrations, the only species of importance are Ià ¢Ã¢â‚¬ Ã¢â€š ¬, I2 and I3à ¢Ã¢â‚¬ Ã¢â€š ¬ [21]. The type and nature of the iodine species present in the iodine-polymer complexes depends on t he nature of the polymer. Many iodine- synthetic polymers complexes were known to regulate the release of iodine like povidone-iodine (Betadine), iodine incorporated resins (quaternary ammonium polymers), iodpolycom complex (vinylpyrrolidone and butyl methacrylate) [7, 18, 19] etc. Medical research of the Sklifosovskiy Research Institute and the National Centre for Toxicological and biological Safety of Medical Products (Report No. 5-10, Jan 11. 2012) [7] demonstrated that the complexed iodine exerted no burning effects on surrounding tissues. However there are some disadvantages associated with iodine-artificial polymers like non-biodegradability, toxicity, expensive etc. which can be overcome by using natural gums. References: J. Home Energy Management System

Home Energy Management System 1) INTRODUCTION Network of electricity that intelligently integrates with users actions connected to network for sustainable, economical and secure supply is called smart grid [1]. Smart grid is called smart due to fast communication and networking capabilities. Smart grid has an important role in energy structure adjustment, coping with climate changes and economic development [2]. Since 1982 energy demand during peak hour is increasing 25% approximately every year [3]. New intelligent devices must be used to fulfill energy requirements. In order to add intelligence new technologies are to be developed. Electric intelligence is only requirement for operational cost and energy consumption reduction [4]. These new technologies should be able to remove peak load and off load power difference, should be capable of making demand-supply curve smoother and should reduce environmental pollution. In smart grid user plays a vital role in reducing and optimizing energy consumption thus improving system efficiency. In smart grid emission of CO2 and household energy consumption is reduced by 9% and 10% respectively [5] In order to improve electricity consumption keeping in view of consumer’s need there should be an optimum solution. Different optimization techniques can be used for energy management. Different technologies like home area networks, home automation, advanced metering infrastructure and bidirectional communication are introduced by smart grid during past few years [6]. Now a days Zigbee and sensor networks not only monitors the quality of power but use powerful strategy for communication and distribution and sale locally generated energy back to grid [7] [8]. Demand Side Management (DSM) system is important utilization efficiency parameters which have been ignored due to complex dynamics of consumption, random behavior of consumers and lack of commutation technology. The advancement in communication technology has revolutionized the power sector and introduced a concept of new modernized electrical system called as Smart grid [9] .The concept of demand side management was first introduced in late 1970’s which reduces GHG, provides reliable energy reducing the electricity cost [10]. Traditional grids consists of DSM but do not provide such type of reliability to users due lack of sensors and inefficient automation and communication. But smart grid is more efficient due introduction of low cost sensors, smart meters and integration of ICT [11]. Challenges faced by smart grid is shown below Interoperability is said to satisfy if multiple communication network coexist in smart grid. Scalability means increasing number of hardware’s in proportion to others but due to increase in demand this become a major issue can be solved by using sensing networks. Smart grid holds different communities and societies which become an issue in order to resolve it integration of power system is required which is done with actuation, security and communication networks. Security is main issue in smart grid because a hacker can interrupt the data of smart grid and easily access to smart meters so this issue can be resolve by modernization the security and by data hiding [12]. The smart grid consists of HEMS that enable demand response and demand side management. On the basis of power supply demand response is responsible for altering and managing the energy and demand side management (DSM) controls the planning, techniques and implementation of policies [13]. There are two major schemes of HEMS one is communication and 2nd one is optimization [14]. Home energy management systems consist of three basic functional blocks [15]: HEMS software Home energy management center(HEMC) Load scheduler. HEMC provides customer’s user’s friendly graphical representation which not only provides assistance to customers but also give them control over various loads using load scheduler. HEMC software uses lab view developer tool that provides necessary information to customers using Zigbee protocol. It has two main sectors a) home tab b) data tab. The information load control, line control and on/off control is provided by home tab on/off control use in switching sequence and its major application is that it is used to detect any abnormality in hardware. The data logging of current and voltage with in time is represented by data tab. Load scheduler considers the bundle of single knapsack known as multiple knapsacks due to which customers become aware of peak load at specific time and interval of occurrence. A load scheduler not only determines the critical and non-critical loads but also time dependent loads. In emergency conditions it also controls various loads. This also stores data of electricity consumption 24/7. Energy management is an organized and systematic coordination of procurement, conversion, distribution and use of energy to meet the requirements, taking into account environmental and economic objectives [16]. Energy management systems are computer aided tools that are used by operators of utility grid for controlling, monitoring and optimizing the generation and transmission system performance. In efficient energy consumption the participation of end user is as much important as the supplier. In modern power supply systems end users’ are provided with RT-feedback (real-time feedback) via different website portals, in-home displays (IHD), or some other feedback viewer device like mobile etc. [17]. Provision of feedback to end user will motivate them to alter their energy usage to minimize the electricity bills. In present smart grid system end users’ are forced for shifting their energy usage on peak to off peak hours regardless of their comfort and life style [17]. All over the world major portion of energy is consumed by residential users, so they can play an important role for energy optimization. Research shows that energy consumption is minimized by 12% by installing energy consumption information system and displays energy consumption of whole unit [18]. Due to advancement in technology the industries has become advance due to which energy demand increases which results in load shedding and blackouts and use of fossil fuel is increases which will finish soon. Now world is going toward non-conventional energy resources like solar and photovoltaic cells but in order to provide information regarding to electricity to customers the smart hems use analogue and digital systems which is an efficient methodology. Home energy management systems play a fantastic role in distribution of energy via conventional grid and homes optimistically. With increasing demand of energy the communication in wired as well as in wireless medium is increased. Internet connection and intranet connection is makes low cost smart homes. Zigbee is one of the devices that uses for communication between smart homes and smart grid. Zigbee alliance introduced Zigbee standard protocol based on IEEE 802.15.4 standard set by IEEE and new standard committee (NESCOM) for low rates wireless personal area network [15]. Zigbee consists of four layers. Zigbee Zigbee alliance platform IEEE 802.15.4 There is a great difference between energy production and consumption, low production cause deficiency of electricity supply. World energy production pie chart is shown as There are many optimization techniques that are used for optimization one of which is knapsack. In knapsack there are many algorithms used for obtaining the best optimized result. In this paper we will make comparative study of some algorithms of knapsack and we will find the best solution obtained from these algorithms. Comparison of different home energy management schemes: Optimization based residential energy management: It is the linear programming model basic purpose of this model is minimization of electricity cost at residential areas [1]. In this scheme a day of 24 hours is divided into equal time slots having equal lengths consists of various prices of electricity like time of use (TOU) tariffs. The objective function shows that with proper scheduling we can reduce the energy expenses by division of home appliances in time slots. Objective function is defined as EI DI Ut St Where I define number of appliances, E defines energy consumption of appliance, J number of days, DI length of cycle of appliance, K number of requests, Ut unit price for slot t and T number of time slots. IN home energy management (iHEM): This scheme use smart appliances like energy management unit (EMU) and wireless sensor home area network (WSHANs) for communication purposes between appliances (IHEMS). It also uses (Zigbee) protocol, wireless sensor networks and cluster tree topology. In this scheme customer may turn on appliance at any moment without being worried about peak hours. In home energy management (iHEM) gives suitable time to customer for appliance use. How iHEM works a request package is sent by appliance to EMU at the start, on receiving the packet EMU communicates with storage system in order to know available energy. Energy management unit (EMU) than communicates with smart meter for current prices. The storage unit send available reply containing information about storage energy when EMU receives packet, it schedules the suitable start time according to iHEM algorithm. It also reduces the carbon emission and energy consumption cost. 2) KNAPSACK: The single knapsack is a problem of combinatorial optimization having objects, values and weights packed into knapsack of specific capacity such that value of object is maximized [19] .The multiple knapsack is generalization of single knapsack problem it is resource allocation problem consists of M resources and set of N objects [19]- [20]. Knapsack problem is basically an items set that have different weights and values. Our choice of item must be in such a way that it should be maximum among the weights of given items. Knapsack allows community to use energy efficiently in order to achieve their goals. It not only minimizes the customer bills but force them to use their heavy appliances other than peak hours. It is estimated that energy demands overall around the world increasing 25 percent per year so its great challenge for us to fulfill the demand [21]. In order to overcome the increasing demand of energy we need to optimize energy usage. Knapsack is one of the optimization technique used for optimizing a problem. 2.1) Types of Knapsack: There are different types of knapsack which are as 1) 0-1Knapsack problem. 2) Bounded Knapsack problem. 3) Unbounded Knapsack problem. 2.1.1) 0-1 Knapsack problem (binary Knapsack): In such case the item is either taken or not taken (accepted / rejected) there are no other possibilities. Suppose a set of ’n’ items having different weights and values say ‘wi’ and ‘vi’, xi is the number of copies of item of set ‘n’. In mathematical form: Maximize ixi Subject to i xi ≠¤ W Here Xi {0, 1} 2.1.2) Bounded Knapsack Problem: In bounded knapsack problem restriction on xi is removed i.e. xi is an integer value in BKP. In BKP restriction is applied on copies of each item of set ‘n’ to some integer value say ‘ci’. Mathematically: Maximize ixi Subject to i xi ≠¤ WHere xi {0, 1†¦ ci} 2.1.3) Unbounded Knapsack Problem: In unbounded knapsack problem no restriction is applied on xi. Mathematically: Maximize ixi Subject to i xi ≠¤ WHere xi ≠¥ 0. 2.2) Algorithms for Knapsack: There are many algorithms for solving 0-1 knapsack problems which are as [22]: 2.2.1) Brute Force: It is a straight forward approach based on statement of problem and concepts’ definition. If there are n items that can be chosen from a set of given items then there will be 2n possible combination of items for knapsack. There is a bit string of 0’s and 1’s, if the string is 1 of an item then it is chosen if 0 then not chosen. 2.2.2) Dynamic Programming: It is a technique in which a complex problem is divided into smaller sub problems. These sub-problems are then solved and are recorded in table. Thus table obtain is utilize to solve original problem. The complexity of this algorithm is O (NlogN). 2.2.3) Greedy Algorithm: This algorithm requires some common sense and problem solving experience for solving problem. It’s a decision making process which may have following steps: 1) Choosing such item that has maximum value in knapsack. 2) Choose item with less weight. 3) Choose item with high value to weight ratio. 2.2.4) Genetic Algorithm: It is an algorithm used to search best solution among different possible solution of a problem. It begins with a solution set and each set is called population. A new population is made from old population by selecting them according to certain specified fitness level. 3) Appliance Usage Control: In [23] consumption of electricity is not always rational it also depends on human psychology factors. Different people use different appliances at different time slots. Now if we consider some appliances we can classify them into following categories: 1) Must Run Appliances: Appliances whose time slot cannot be changed and they must run comes in this category. 2) Fixed Run Appliances: Appliances that run only once a day at specific time. 3) Flexible Appliances: Appliances that can be run at any time in a day they don’t have fixed time. Some of the household devices are shown below: Oven LampRefrigerator Fan Considering human psychology factor the use of appliances can be categorize as: 1) Emergency Use: Appliances that use in some sort of emergency. 2) Welfare: Appliances that use for welfare 3) Enjoyment: Appliances that use for enjoyment purpose. Taking an example of personnel computer that is used by student for his working purpose or for emergency while an adult use it for enjoyment purpose. In order to make decision on which appliance should be used during peak hour follows analytic hierarchy process. Bibliography

The Ants and The Bees: Comparing the Social Structure of these Colonia

A queen, drones, and workers share a common place in life. They all live in a bee and ant colony striving together to accomplish their specific jobs. Living in a world where females are your workers and protectors of the queen, and drones are used to multiply the colony, and then die. When you look passed the buzzing of ants, you see a very sociable creature working like a well oiled machine. When you hear the dirt on the bees, they may look sociable, but deep down, it’s another world. A world where queens kill sibling sisters, and the drones are ejected out into the cold to starve just for the sake of their colony. Where females defend the nest and die 100% of the time after the sting. Bees and ants, both considered sociable insects, have a very different view on sociability. The colonies of ants and bees are broken down into several different categories. The top of the chain consist of the queen in both colonies. The colony depends on the queen to produce the eggs for their survival. This is the only function for the queen. She is able to choose what gender the eggs will become by fertilizing or not fertilizing them. She is capable of keeping the balance of the colony in check by choosing the gender of her eggs. Unfertile eggs become the drones of the colony, where fertile eggs becomes the female workers. The males, or drones of the colony serve only one purpose, and that is to fertilize the eggs. The drones do no maintenance work, no tending to the colony, no foraging, and they shortly die after mating with the queen. The workers of the colony are all female and mostly foragers, but some do have different task within their society (Stanger et al., 1971, p.10-11). The same can be implied to the ant colony which is made up of ... ...ned (Layton, 2008, p.1). They clamp on with there mighty jaws, rotate in a circle, and sting their victim while still clamped on. Some ants do not have a stinger. They simply drop poison unto their victims skin. Bees and ants live in a social nest setting, where there is a queen, drones, and workers. At first glance they may seem to be almost the same, one with wings and one without. Storing honey for rougher seasons and times, protecting by stinging, and having different types that are more harmful than others. At a closer look, ants are more the social creatures than bees. Living with more than one queen and sharing the neighboring colonies workers. Bees seem to live a more a closed off, mobster type. The queen killing off her sister rivalry to rule over the colony. Bees and ants may be similar in social structure, but different when you get down to the dirt.

Kurt Cobain :: essays research papers

Many people have their own theories about the 5th of April is 1994, for those of you who don't, know that this is the day Kurt Kobain's body was found in the garage of his home. Some critics have come to the conclusion that Kurt had tried to make himself a legend by killing himself. But such ideas seem to overshadow the tragedy of the death of one the most popular and influential musicians of the 1990's. (Melody Makers, web2.epnet.com 12/08/2001) There is no question of Kurt Cobain's death. Many people think he was murdered, many people think it is pretty impossible to shot yourself in the head with a shotgun, but truly it isn't. Kurt Cobain was found on the floor of his garage, when he was found he had been dead for some time from a single "self administered shotgun wound to the head." Around him on the floor were some personal possessions and a had written suicide note which read "I have it good, very good, and I'm grateful, but since the age of seven I've become hateful towards all humans in general" (Iain Shedden, "Smells Like a Dead Man"). Cobain had grown up in a small town named Aberdeen. Washington. His childhood was disrupted by the divorce of his parents when he was eight. The success of Nirvana gave Kurt the authority to be the "spokesman" of his generation. Nirvana's music gave way to hundreds of other underground bands, but with this success came the media's attention and the almost total loss of privacy that came with it (Kurt Kobain's Obituary). Kurt had secretly suffered from an illness that caused severe stomach pains for more that seven years, which caused him to contemplate suicide almost everyday. This constant abdominal pain led to deep "melancholic depression verging of schizophrenia, and frequent bouts of narcolepsy" (Kurt Kobain's Obituary). Doctors were of no help to him, so he found escape in heroin. For years he fought his addiction, but compared to the severe stomach pains, heroin was unimportant (Kurt Kobain's Obituary). Kurt Cobain's wife, Courtney Love, and their daughter, Frances, brought some real happiness into his life. But the media's constant prodding and increasingly frequent period of depression finally pushed him over the edge. In the end, death was the only thing that he saw that could relieve his pain (Kurt Cobain's Obituary). A famous photograph of Kurt Cobain after a concert in 1991 shows a disturbed young man trying to gain control of his life. Rehabilitation Of Multi Level Surgery Health And Social Care Essay

Multilevel surgery ( MLS ) is employed to pull off multiple joint malformations in kids with intellectual paralysis. No studies exist associating to the community-based rehabilitation issues that affect the MLS results. Therefore, we retrospectively analysed instance studies of 3 kids who had undergone MLS comparing their pre and station operative pace and motor maps utilizing Gross Motor Functional Measures ( GMFM ) and Edinburg Visual Gait Scores ( EVGS ) that are simple clinical result steps instead than instrumented pace analysis. All kids achieved independent standing within 1 twelvemonth postoperatively. They all experienced neuropathic and complex regional hurting syndrome ( CRPS ) following surgery and later developed stringency in iliopsoas, hamstring and IT bands. Each had trunk hypotonus, which hindered nucleus stableness and compromised their pace and position. It was observed that bole hypotonus, CRPS, nervus stretching hurting, return of stringency and decreased nucleus stableness negatively affected pace. EVGA appeared more sensitive to observe alterations in functional betterment than GMFM following MLS, hence EVGS can used in the community to quantify alterations in pace after MLS.IntroductionCerebral paralysis ( CP ) , a non-progressive cardinal nervous system ( CNS ) disorder bounds normal motor development and contributes to postural abnormalcies ( ref ) . Primary damages seen in kids with CP are increased musculus tone and failing ( 1 ) leads to development of myostatic contractures and secondary skeletal tortuosity malformations ( 2 ) . Such contractures are typically managed with surgery in order to heighten mobility and map. Multi degree surgery ( MLS ) , an orthopedic attack to surgically realine unnatural articulation angles is performed with an purpose to bettering pace in ambulatory kids with CP. It has been defined as a process affecting one or both limbs, including at least three articulations ( 3-5 ) . Following MLS, important betterment in the pace and kinematics are observed with instrumented pace analysis, which include care of articulatio genus musculus power ( 6 ) , increased power coevals at hip and mortise joint, increased pelvic joust and articulatio genus scope of motion ( ROM ) and improved ankle map ( 7 ) , gait velocity, stance limb stableness and an unhampered swing stage of the opposite limb ( 8 ) . However, instrumented pace analysis is non executable in the community, hence, clinician require simple and cost-efficient and simple result steps. Gross Motor Functional Measures ( GMFM ) and Edinburg Visual Gait Score ( EVGS ) that are simple to measure hold been found to possess good dependability and cogency in kids with CP ( 9-12 ) . However, to our cognition there is no literature analyzing issues related to the postoperative rehabilitation following MLS within the community such as postural tone and return of stringency for kids with CP indicated by GMFM and EVGS. A s there is limited figure of kids with CP treated with MLS we retrospectively analysed 3 instance studies and investigated the factors that influenced results whilst being treated within a community scene.Case studiesFrom 2005 to 2006, three male kids diagnosed with spastic diplegic CP from Cambridgeshire Community Services had MLS. They underwent extended postoperative physical therapy for at least one twelvemonth. Two hebdomads after MLS, they were discharged from infirmary and re-admitted 6-8 hebdomads subsequently for 2 hebdomads of intensive physical therapy intervention. Initial rehabilitation in the community was commenced from 3rd hebdomad and focused on obtaining full scope of inactive motion, in add-on to deriving strength and motor control in the operated limbs. Our kids were treated twice hebdomadally for the first 4 months, whilst the frequence of farther therapy ( for up to 1 twelvemonth ) was dictated by the persons rate of patterned advance in motor public presentati on. Subsequently, they were reviewed at 3-4 months intervals and ongoing rehabilitation was directed toward specific job work outing as they arose. All kids reached their pre-operative functional degree within 12 months following MLS. All these topics were assessed in the community utilizing GMFM and EVGS prior to and 4 old ages after MLS. ( table- 1 ) .Case 1JD, a 12 twelvemonth old athleticss oriented male child with normal evident knowledge and go toing mainstream school had undergone strength continuing MLS integrating bilateral femoral derotation osteotomy, bilateral medial hamstring release, bilateral rectus femur transportation, bilateral prolongation of gastrocnemius and prolongation of right adductors. Community-based physical therapy rehabilitation programme was commenced from the 3rd hebdomad post- surgery. Full ROM was achieved in all articulations after 2 months but deficiency of hip and knee active extension at the terminal scope associated with increased sympathetic a ctivity and hypersensitivity in the pess limited the gait of rehabilitation. He suffered from complex regional hurting syndrome ( CRPS ) between the 2nd and 4th month and was treated with anodynes. He achieved independent standing after 5 months, progressed to walk without splints ( immature pace ) by the 7th month whilst return of bilateral stringency in the hamstrings and TA was noted at 10th months. JD continued to hold trunk hypotonicity and his returns of hamstring and ( iliotibial set ) ITB stringency and hip extensor failing were managed through supervised exercising programme at place and school. JD discontinued leg splints and his limb length disagreement ( LLD ) was managed with a shoe rise. A important betterment in pace and position following MLS was achieved and this was supported through EVGS mark and the disappearing of spasticity reflected by normal articulatio genus and mortise joint physiological reactions.Case 2JS, a 15 twelvemonth old male child with mild larning troubles go toing particular school had undergone a individual event MLS integrating bilateral Intramuscular psoas prolongation, adductor longus prolongation, bilateral hamstring lengthening without rectus femur transportations, bilateral transdermal prolongation of the Achilles sinew. After initial discharge from infirmary he was readmitted for 2 farther hebdomads due to unbearable nervus stretching hurting and was treated with anodynes. He was really dying, lacked dark slumber and his parents found it difficult to get by up with his hurting. Despite medicines his sudden and self-generated pes hurting and hypersensitivity prevented his active engagement in therapy Sessionss. Consecutive bivalved projecting to stretch hamstring musculuss was performed at the beginning of the 4th month from which point the relentless hurting decreased but persisted till the 6th month. JS stood and walked independently by the seventh month. Trunk hypotonus limited his unsloped position and a subseque nt development of hamstring and ITB stringency was noted at the 9th month. Despite regular therapy hamstring stringency has progressed farther but he has maintained independent mobility. The GMFM station surgery mark showed little impairment but the EVGS demonstrated an improved pace mark.Case 3MT, a 13 twelvemonth old male child with limited motive go toing mainstream school had undergone initial left femoral derotation osteotomy a month before MLS. MLS incorporated left hamstring prolongation, left rectus femur transportation, left transdermal TA prolongation and right vulpius slide. Following surgery MT experienced leg nervus stretching hurting, CRPS in the left pes and he was treated with anodynes. He was made to partly weight bear with splints at the tenth hebdomad and he walked within a parallel saloon at the beginning of the 6th month. Independent standing was reached at 7th month but unsloped position and independent mobility was delayed due to miss of full articulatio genus extension scope, bole hypotonus, hapless nucleus stableness, LLD and sustained failing in hip, articulatio genus and mortise joint musculuss. A spinal stableness wrap and an exercising programme to stretch tight musculuss to better nucleus stableness provided limited betterment in position. MT had hapless ankle stableness due to pick pronation after MLS and this was managed with AFOs. After 1 twelvemonth he developed hip and articulatio genus flexor stringency and began to utilize an cubitus crutch.Discussion:Muscle tone: All three kids had trunk hypotonicity and they re-established their crookback position after MLS. Spinal humpback is positively associated with posterior pelvic joust and the corresponding development of stringency in hip and articulatio genus flexor musculuss. Despite regular physical therapy, topics developed moderate stringency in both hamstring and iliopsoas musculuss. MLS is focused upon rectification of spasticity-related biomechanical abnormalcies and it wa s ill-defined that bole hypotonus and its consequence on position and mobility had been taken into consideration before MLS. Borghuis et Al ( 2008 ) suggested that reduced nervous thrust leads to the development of active stiffness and co-contraction in bole musculuss, thereby impacting nucleus stableness ( 13 ) . It is proposed that CP kids who have trunk hypotonia set up postural stableness through selective stiffness of hip flexors muscles that triggers perennial stringency in iliopsoas followed by hamstring musculuss within 1 twelvemonth after MLS. Consistent with this hypothesis we observed that the badness of bole hypotonus has a negative influence on rehabilitation result of MLS reflected through decreased pelvic stableness and hapless position. It is interesting to observe that one topic demonstrated normal articulatio genus and mortise joint dork physiological reactions following MLS that incorporated musculus angle resetting, bespeaking normal musculus profile in spastic m usculuss. This observation supports the impression that spasticity, a gravitation dependent CNS characteristic tends to switch its kineticss if musculus orientation changed. It is further supported by the observation of Patikas et Al ( 2007 ) that the EMG form of lower limb musculuss were comparatively normalised following MLS ( 14 ) . However, this was non the instance in the other two topics in which there were no alterations in stretch physiological reaction responses. This could be due to inadequate prolongation of hamstring that compromised the orientation of musculus pull angle following MLS, hence no alterations in musculus tone. Stringency: Complete articulatio genus extension was non surgically obtained in two kids and undistinguished betterments in articulatio genus extension was attained through consecutive casting. This resulted in trouble in accomplishing unsloped position and they retained little flexure in the hips and articulatio genuss during weight bearing. This places quadriceps musculuss in weak and automatically deprived place against the sawed-off hamstring musculuss. It may besides restrict the chances for sarcomeres lengthening of developing and wasted vastus medialis due to deficient articulatio genus extension scope even after MLS. These topics took longer to accomplish independent mobility, developed hamstring stringency and scored lower in EVGS postoperatively. This is in understanding with Gannotti ( 2007 ) , who reported development of stringency and impairment of motor accomplishments following MLS in kids with CP ( 15 ) . The other topic who had full articulatio genus extension in one leg and mild articulatio genus flexure in the other leg showed important betterment in pace position and map, proposing that accomplishing complete articulatio genus extension surgically is a important factor for better long-run MLS results. On the contrary, Adolfsen et Al ( 2007 ) reported hyperextension of articulatio genus in mid-stance following MLS in his survey and questioned the indicants for hamstring prolongation ( 16 ) . Our observation back up the position that complete articulatio genus extension is indispensable for the better result following MLS. Development of ITB stringency in MLS topics has non reported earlier and we found that all our topics developed stringency in ITB along with hamstring musculuss which had an inauspicious consequence on their position and pelvic stableness. Previous research in both normal and athletic topics reported back hurting, ITB clash syndrome and other biomechanical abnormalcies followed by ITB stringency ( 17 ) , Therefore, we extend this to kids with CP & A ; acirc ; ˆâ„ ¢s after MLS and suggest that integrating ITB stretching from the early phases of physical rehabilitation following MLS is be important to gait sweetening. As kids grow older, increased return rate of imposter myostatic contracture due to growing jet and the corresponding residuary pathological force generated in their weak spastic musculuss was observed. It is critical to keep musculus length and go oning regular stretching exercising programme to understate the return of stringency. Pain: Complex regional hurting syndrome ( CRPS ) is a common postoperative complication in lower limb surgery ( 18,19 ) . Two kids displayed CRPS symptoms which limited their cooperation and early weight bearing. All kids experienced neuropathic hurting and two of them experienced self-generated leg hurting discharge associated with intermittent cramp that lasted for at least 4 months following surgery. Lauder GR and White MC ( 2005 ) in their instance surveies acknowledged CRPS following MLS saying that TENS was a utile mode to pull off neuropathic hurting ( 20 ) . However, the current survey topics were neither orientated nor introduced to TENS postoperatively alternatively used anodynes for hurting alleviation. TENS could be a utile and cost effectual intervention mode to pull off postoperative hurting and CRPS in pediatric population in a community scene and this demand to be studied farther. Cognitive degree of topics was a factor finding the MLS result. Poor knowledge negatively affected their therapy programme conformity, motive to fall in with equals, hurting tolerance and engagement in early dynamic strength preparation exercisings. For case, a kid who was immature and had normal knowledge showed better consequences than an older opposite number with mild larning troubles. Outcome steps: All topics showed little alterations in GMFM mark but the positive betterment is relatively higher with EVGS. Although the GMFM has good inter and intra-rater dependability in CP ( 21,10 ) , the single dimensions of GMFM were non sensitive plenty to observe alterations in motor map in kids with CP ( Josenby, 2009 ) . Our observation suggests that GMFM may non be a suited tool to quantify the betterment after MLS for those who already scored more than 90 % preoperatively. We found EVGS a clinically utile and easy tool to objectively detect alterations in pace parametric quantities within the community. Therefore we propose that utilizing EVGS as an result steps in MLS may help the clinicians in the community to quantify the betterment in pace. Further research is required to find the sensitiveness, dependability and cogency of EVGS in MLS.Decision:From this instance analyze the writers observed that bole hypotonus, return of stringency in hamstring, IT set and iliopsoas and CRPS has an inauspicious consequence on kids who had undergone MLS. Despite this, all the kids showed betterment in mobility and the EVGS found to be a utile clinical appraisal tool than the GMFM to quantify gait betterment in the community. These decisions were drawn from a little group of kids, hence, farther probe of these factors and the usage of EVGS in MLS is required to find the rehabilitation result in the community. Table. 1. Christ the Savior Essay

Augustine alikek birth on 13 November, 354, at Tagaste. He was born in a respectable, middle class family. His father, Patricius who was one of the curiales of the metropolis was still a pagan. However, his wife, Monica was an ardent Christian and her devotion and corporate trust brought her husband too to follow Christianity before he died in 371. It was in a Christian automatic tell aparter machine that Augustine converged his formal education. . It is said that at once when he was very ill, he asked to be baptize, only, as he recovered degraded he did non want to receive the sacra handst then.Augustines friendship with men of payer gave him three great ideas which he deeply engraved upon his soul a Divine Providence, the future life with howling(a) sanctions, and, above every, Christ the Savior. In vindications I, he writes From my tende simplicity infancy, I had in a style sucked with my mformer(a)s milk that come upon of my Savior. Thy Son I kept it in the reces ses of my total and all that presented it egotism to me without that Divine nature, though it might be elegant, well written, and eve replete with truth, did non altogether concord me away (Augustine)Augustine was doing well in his tutor and his father Patricius wished to send him to Carthage to study forensic medicine, but it took Patricius homosexualy months to make the necessary arrangements. Augustine, in the mean snip had to spend other year at his school at Tagaste in total idleness. It is said, an idle head is the dimmoralitys workshop, (Proverb) and Augustine plunged himself wholeheartedly to the pleasure of the image. Even when he joined Carthage, the legion(predicate) seductions of this great city which was fractional pagan pulled him towards untoward things. He had a great desire to excel in everything, crimson evil. Very soon he make his acknowledgment to his mother that he had a son outside wedlock. Augustine had a certain dignity some himself even in his fall. By the age of xix he had got over his problems and in 373, when he read Ciceros Hortensius he imbibed a sock of the wisdom which Cicero speaks of. He was at a time heart and soul into philosophy. But this faith underwent a great crisis when he along with his friend Honorataus became prey to the Manichaeans. He was interpreted up by the promise of indigent philosophy. They claimed that they had prepargon a scientific business tattleship to nature and Augustines inquiring and searching mind was attracted towards it. Moreover it was alluring as it propagated a moral irresponsibility resulting from a doctrine which denied liberty. Now Augustine devoted himself richly to the sect. He read all their books and it was during this outcome that he developed his literary faculties. When his student days at Carthage were over, instead of practicing forensic, he re crappercelled to Tagaste to teach Grammar. He was an excellent teacher and his students adored him.One of his stu dents, Alypius followed him to the wrong cart track but later along with him was baptized at Milan. His mother Monica was so overrule about him joining the Manichaeans that she would not excite him in the house, but a not bad(predicate) bishop t senior her, the son of so many tear could not perish. ( Bishop Ambrose) Augustine went buttocks to Carthage where he taught the students rhetoric. He pursued liberal arts and his priming coat sharpened and gained full maturity at this time. His origin works on aesthetics were completed around this time and it was this instant that he started deploring Manichaeism. jibe to him Manichaeism philosophy They break everything and built up nothing. (Augustine, Confessions) Their arguments he found feeble directly and he could dumbfound no acquisition in themscience in the modern sense. (Augustine, Confessions) When he met Faustus of Milve, the Manichaen bishop, the trance was broken and he unexpended the sect. He had been with them for nine years. It was in Italy under the work of Bishop Ambrose, whose kindness made Augustine attend his sermons that he embraced the faith. For three years his mind underwent a struggle. archetypal of all he turned towards the philosophy of Academics, then neo-Platonic philosophy. Finally it was the practice session of the Holy Scriptures which told him that messiah Christ was the honest Savior. In 386 Augustine took himself to the country side of Verecundus, where he devoted himself to true philosophy which he considered was all about Christianity. He slow studied Christian doctrine and became present with it. In his book, Against the Academics Augustine had described the ideal serenity of this existence, enlivened by a affectionateness for truth. He held philosophical conferences where they discussed truth, certainty, true joy and graven image and soul. In 387, during Easter time, Augustine trus tworthy the sacrament of Baptism. In the Dialogues he tells us abou t his conversion, the progress he made in the Faith, his conferences with his friends on the Divinity of Jesus Christ, the transformation of his own soul, the calming of his passions and his determination to choose wisdom as his spouse. frankly his philosophy is neo-Platonic, but he never hesitates to subordinate his philosophy to religion. He was a Christian first of all, and he tried to kick downstairs harmony between the two doctrines, but it was the fundamental points of Christianity that he sought. First the great mystery, the word made flesh and then love, resting on the basis of humility. He sought Divine grace in Christian baptism. g-forcegh he and his friends determined to retire to Africa in solitude, it was his mother Monica who retired to Africa in solitude. Augustine was in Milan writing his works, On medical specialty and On the Immortality of the Soul. In 387, his mother Monica left this world to be with her maker.Confessions IX tell us about Augustines hearta che and her saintly death. In 388 Augustine returned to Tagaste, here he cherished to lead the perfect life, so the first thing he did was to rat all his worldly belongings and outpouring the proceeds to the poor. He then went to his terra firma and at that place with his friends lived a life of prayer, and poverty. It is during this time that he wrote De Vera Religione, De Genesi contra Manichaeos and De Magistro. Augustine never thought of go in priesthood, however he was called to hippopotamus by one of his friends who wanted him to pray over him.Augustine was praying in the church service when the people gathered round him and begged the bishop of Hippo Valerius to ordain Augustine as a priest. He was ordained in 391. He went back to Tagaste to resume the religious life and there he preached for five years as a priest. He fought against Manichaeaism now and his conquest was great. Valerius at this time was getting old and he took the permission of Aurelius, Primate of Africa to guide Augustine preach with him as coadjutor. Augustine was Bishop of Hippo for thirty four years.Augustine now combined his untaught duties with the austerities of the religious life he lived in a monastery with his clergy and they all bound themselves in poverty. He thus founded the order of monks who had their readying with him and then went out and founded monasteries throughout Africa. Augustine was the withstander of the truth and so he combated the Manichaean, the Donatists, and the Pelegian Controversies. In 426 the holy Bishop of Hippo, at the age of seventy-two chose deacon Heracles as his successor and go away from this land on 28 August, 430. He was seventy six years old.In Confessions a notable thing about it is Augustines confessions about his sexual career. As a young man, he was sexually alive, lived out of wedlock with his mistress and had a son through her. Critically observing, he was not much different from other youngsters but the difference lay in that his sexual exploitations were a source of enormous pain to him. This may be the reason why he emphasis on it. He describes his sexual impulses in the prejudicious form. Thorns, (2. 3) desire in the mud (2. 2) an rotate sore (3. 1) he feels he cannot mince his sexual feelings until he has help from God.Augustine was not unique in this negative attitude. on that point were many wealthy young men in Rome who gave away everything they had when they born-again to the faith. Confessions represent a literary milestone. According to A. F. West it dates the entrance of a sweet kind of autobiographythe autobiography of introspection, the self registered record of the development of a tender soul. In his Retractions Augustine presupposes of his Confessions, The thirteen books of my Confessions whether they refer to my evil or good, praise the fairish and good God, and stimulate the heart and mind of man to approach unto Him.And, as far as pertaineth unto me, they wrought thi s in me when they were written, and this they work when they are read. Augustine give the axely meant that Confessions was not a book of his confessions only, but one which the commentator should debate upon and praise the one who is to be praised, the just and good God. He says that his account of his prehistorical sin is to draw attention to the love of God I will now call to mind my past foulness, and the sottish corruptions of my soul, not because I love them, but that I may love Thee, O my God.For love of Thy love do I it. (2. 1. 1) ( Augustine, Confessions) His intention of his book is to focus the readers attention on God and not on his life. Why, therefore, do I place before Thee so many relations of Things? that I may change my own love and that of my readers towards Thee, that we may all say, massive is the Lord, and greatly to be praised. (11. 1. 1) (Augustine, Confessions) This was the routine of his book. Yet it would be wrong to say that he did not confess and there is no repentance. But yet do Thou, my most secret Physician, make clear to me what fruit I may get out by doing it. For the confessions of my past sins, which Thou has forgiven and covered, that Thou mightest make me happy in thee. (10. 3. 4) Bonner comments The Confessions first and foremost a work of praise, as is made clear by the curtain raising spoken language, taken from Psalm 145 Great art Thou, O Lord, and greatly to be praised great is Thy power, and infinite is Thy creation. Confession, then, for Augustine, is primarily confession of praise and thanksgiving to God for his mercies.Confession of sin has its place, but in relation to Gods grace and not in its own right. In conclusion, we can summarize his book by his words For Thou hast formed us for Thyself, and our paddy wagon are restless till they find rest in Thee. (Augustine, Confessions) He is public lecture about his spiritual journey when his heart was restless till it found rest in God.Works Cited Augustine. The Norton Anthology of World Literature. Glamourous

Dylon de Leon ENGL 100-08 QFRA 6-11 (pg. 186) Feb. 22, 2013 glamourous What is glamour? How does it differ from looks, fame and fortune? In split 12 of the article, the first few sentences be actually effective. They use 3 people that a sens of people know today and go swell to the point about who they argon in the governmental world. This is an example of Givhans strategy that was shown to be effective. Givhan uses a few strategies throughout his article. One of the specific strategies was using celebrated and glamourous people that we assemble today as rich and famous and see as having fame and fortune. other good strategy that he uses is pointing out the exhibit between actors that they are not all what they take care and that who they real are isnt who we see in public, and that what we are really seeing and who we really see they are is safe an act. He points out that really glamour makes people forget about who they are and make everything seem so beautiful in the world.Givhans claim is evidently that the people who we see living their lives full of fortune and happiness is solely not correct. He claims that glamour is retributive simply not all that great. Givhan states that glamour is situated in the now while tranquil is a beat ahead. This claim makes perfect sense because glamour is just something that is seen in the moment and not in the future. placid on the other hand is something that is seen outside of what is on the extended screen, therefore is seen in more than than just the present. It may just be the way that I look at it that makes sense. You can tell the deviation between Glamour, Charisma, and cool, by what they show and what makes each indication what it is. Glamour is a sort of fairytale, Charisma is personal, and cool is just, well, cool. If someone was to want to be cool, I would apprize that they do something to inspire people as a famous person. Cool isnt about having much of stuff and looking good, there is a big difference. To be cool, you have to show the people that you are cool and show the people you are more than just good looks.
